When we feel trapped, like there is no way out, we get depressed (“freeze”). Thus, the nervous system plays an important role in both anxiety and depression. You can see examples of how this freezing behavior over time will create an experience of “learned helplessness.” In many ways, this position is almost identical to the subjective experience of clinical depression. However, considering that many other factors besides allelic variants may contribute to the final phenotype of “responder” or “nonresponder,” we cannot assume a one-to-one relationship between genotype and phenotype. Moreover, we must take into account that, like the pathophysiological mechanisms of complex psychiatric disorders, different genes are interacting and modulating each other in drug response, in addition to environmental factors. Nevertheless, the field of pharmacogenetics is expanding rapidly, and the elucidation of the disease processes through genomics, the identification of novel drug targets, and the subtyping of patient populations are all ambitious methods that may help us individualize pharmacological therapy.

In the EPM, these mice exhibited increased time in the open arms, more open-arm entries, and reduced time in the closed arms (Fig. 5l, m). They also displayed higher sucrose preference in the SPT (Fig. 5n) and reduced immobility times in the TST (Fig. 5o) and FST (Fig. 5p). Thus, while several PROs and screening tools for depression exist to assess depression in pwMS, many clinical practice guidelines may not reflect current clinical practice and use of screening tools. Several causes may increase the risk of developing depression, including biologic, psychologic, and social factors (Fig. 1). For example, the stage of MS may influence the prevalence of depression, with the highest prevalence of depression in secondary progressive MS (31.5%), followed by primary progressive (25.8%) and relapsing-remitting (23.2%) 11. Similarly, a recent systematic review and meta-analysis found that the prevalence of depression was higher in people with progressive MS (19.13%) than in people with relapsing-remitting MS (15.78%) 40.
SCN lesions and conditional knockout of Bmal1 induced circadian rhythm disorder
This study provided new insights into the network structure of anxiety and depression in patients with CVD. These identified symptoms may be potentially central nervous system depression effective targets for preventing anxiety and depression in patients with CVD and may provide treatment strategies for these patients. Several limitations of our study should be acknowledged when interpreting our results.

This study conducted network estimation, centrality index analysis, and stability estimation. To evaluate the importance of each node (symptom), three key centrality indicators were calculated; strength, closeness, and betweenness, which mainly assessed the importance of nodes based on strength. The bridge centrality index was calculated to identify the bridge symptoms of anxiety and depression, and its accuracy was evaluated by calculating the 95% confidence intervals (CI) of the marginal weights. A stability evaluation was conducted using the discarded subset guidance of the case studies (2500 guidance samples) to calculate the stability coefficients related to strength centrality.
There is no doubt that the monoaminergic system is one of the cornerstones of research, but any hypothesis for the pathophysiology of depression must take into account the many interactions with other brain systems and the complexity of the regulation of the CNS function. In spite of all the progress that has been achieved in the last decades, we must be aware that there are still today considerable problems in understanding and treating severe depression, and knowing the cause of treatment-resistant depression. The influence of chronic stress and adverse life events on the development of depression has been subject of numerous investigations and the work has been influenced by studies of the somatic and endocrine consequences of stress in animals (see reference 15 for a review). Pharmacologic and non-pharmacologic therapies are widely used to treat depression in pwMS. However, further research is needed to support or refute the effectiveness of many treatments in the MS population 81. Currently, there is no gold standard, single treatment for the management of depression in MS 19. Combination therapy may be beneficial; however, further research is needed to determine efficacy, safety, and feasibility 19. The PHQ-9 is a self-administered questionnaire consisting of nine questions to assess depression 77. The PHQ-9 can be administered by any provider who knows how to interpret and score and can be administered as needed but not more frequently than every 2 weeks 82.
Understanding Depression in People Living with Multiple Sclerosis: A Narrative Review of Recent Literature
Barbiturates are drugs typically used to treat anxiety and sleep disorders. The overuse of depressants can lead to symptoms of CNS depression, including slowed reflexes, lightheadedness, fatigue, and difficulty breathing. Depressants affect GABA, an inhibitory neurotransmitter that slows down activity in the brain. When severe, CNS depression caused by substances such as opioids, alcohol, barbiturates, benzodiazepines, and sleeping medications can be fatal. Depression has a profound impact on pwMS (Fig. 2) as it can negatively affect areas such as treatment adherence, symptom severity, QOL, disability, and functional outcomes. The etiology of comorbid depression in MS is not fully understood although the causes are believed to be multifaceted because of the complex nature of the disease.
This increases the heart rate and rapidly pumps blood to our muscles in order to prepare for “fight” or “flight.” While this is occurring, blood flow is moved away from our internal organs and brain in order to prioritize action, but this also makes thinking clearly and/or critically VERY difficult. In the majority of cases, children in our study were able to be managed at home under the supervision of an adult. However, it is important to note that isolated cases of central nervous system depression following ingestion of an alcohol-based hand sanitiser have been described in other countries, including the US 8. This highlights the importance of storing topical antiseptics, including hand sanitisers, out of reach of children. This is particularly important for younger children who are more likely to be unintentionally exposed than older children and are at greater risk of toxicity. Other measures to mitigate child exposure include keeping hand sanitisers in their original labelled containers to reduce the risk of accidental ingestion.
